When making a decision about surgery, your doctor needs to know all the factors that may influence the outcome of your procedure. If scheduling surgery, it may be useful to bring a personal calendar or day planner so that you can note dates/times for surgery and any additional pre-operative testing that may be required. Scheduling certain procedures requires preparation and careful consideration to achieve the best results.
*It is your responsibility to understand your own health insurance benefits.* Co-pay is due at time of service. The amount a patient may owe depends on their insurance plan, deductible and co pay obligations. If you cannot make your appointment, please give the office at least 24 hours’ notice so we may accommodate other patients.
